If you registered for an enhanced ticket or a reserved table of 6, please note the following:
- The event will take place at the Rock City Events Center (Mosaic Church) at 6221 Colonel Glenn Rd A, Little Rock, AR 72204
- The event will begin at 12:00pm, but we ask that you arrive at minimum 15 minutes early to get checked in, grab your lunch, and find your seat. Doors will open at 11:30am.
- Please read the following safety guidelines:
  - Your safety is our top concern. As a reminder:
  - All attendees will be required to wear masks unless eating or drinking.
  - Each attendee will be asked to sign in at the beginning and be asked standard questions before entry. Your temperature will be taken and recorded.
  - Hand sanitizing stations will be available through-out the venue, and bathrooms are sterilized on a schedule deemed appropriate by the Arkansas Department of Health.
  - Your lunches will be individually boxed. Each attendee will be spaced out to allow you the comfort to enjoy the program while you eat your lunch.

All persons attending the Cultivate event, sponsored by the Little Rock Chamber of Commerce, acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing; and further acknowledge that Little Rock Chamber has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19; and further acknowledge that the Little Rock Chamber cannot guarantee that attendees will not become infected with the Coronavirus/Covid-19;and understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others. The attendees agree to comply with all safety precautions in place at the event location and assume the risk of contracting Coronavirus/Covid-19

All attendees attest:

* They are not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

* They have not traveled internationally within the last 14 days.

* They have not traveled to a highly impacted area within the United States of America in the last 14 days.

* They do not believe they have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

* They have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities.

* They are following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

The attendees hereby release and agree to hold the Little Rock Regional Chamber of Commerce harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the Little Rock Chamber of Commerce in relation to this event. I understand that this release discharges the Little Rock Regional Chamber of Commerce from any liability or claim that each attendee, their heirs, or any personal representatives may have against the Chamber with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, the Cultivate event presented by the Little Rock Chamber.
